Traveled from Africa to treat facial pain. I had pain on the right side of my face start while working in Tanzania. The local doctors weren’t sure of the cause and I took advantage of a trip to the USA to seek other opinions. From the moment I walked in, this office was different than any dentist’s office I’d ever been to. The atmosphere and all of the staff were so warm, friendly, and positive. I’ve had anxiety about the dentist for years; my teeth were so sensitive even simple fillings would hurt so much. I’ve now had upwards of 10 fillings done and Dr. Korwin has not hurt me once! He knows exactly what he is doing; I’m completely numb every time and don’t feel a thing. And he is so friendly and caring and just makes you feel so safe. I am not scared of my dentist any more, I love him! Would recommend him to anyone with anxiety or without. :)

I had teribble teeth and one day I decided that I should go ahead and schedule an appointment, like a lot of people I am affraid of the dentist. Today I found out that there is such thing as no pain dentist. I am extremlly happy with my new smile in only one visit. Until I came to Dr. Korwin’s office, I was always in the dark about my oral hygiene. Before I came to Dr. Korwin, I had numerous problems with my teeth and gums. Unlike Dr. Korwin, my previous dentist was extremely primative in his knowledge and technology. I never had an oral cancer screening, painless fillings and injections. All I can say is if I had to rate Dr Korwin on a scale of 1 to 10, I would gladly give him an 11! My experience at his office was great. He explained everything in a way that was easy to understand and didn’t talk down to me. His amazing staff was a pleasant as can be and everyone worked as a wonderful team. I felt he wasn’t trying to “sell” anything to me, and I trusted everything that had to be done. unbelieveable work When i first went to see Dr. Korwin i was in pretty bad shape. I did everything possible to hide my smile. I was embarrassed to even let them see my teeth. A Dentist You Can Trust I have been a patient of Dr. Korwin’s for about 25 years. He is always on the cutting edge of denistry and technology providing th best of care. Any treatment he has recommended has resulted in my continued health and well being. Now my entire family is in Dr. Korwin’s care! Even when I lost my dental insurance, it wasn’t worth the risk to try another dentist. Dr. Korwin’s staff is also top knotch! Awesome Dentist and Staff!!! I haven’t been to the dentist in many years 15 to be exact I called 1800dentist and they recommended Dr. Korwin. I brought my son first because he has had so many bad experiences I figured he be my best test. He was so happy and content with Sally I decided to make an appt. for myself. I have had most of my work done by Maria and she is fantastic. All the staff here are very sweet and I have not had one bad experience here. Dental X-rays can predict fractures

A new study that shows that it is possible to use dental X-rays to investigate the bone structure in the lower jaw, and so predict who is at greater risk of fractures in the future. Published in the journal Bone, the results were also mentioned in both Nature Reviews Endocrinology and the Wall Street Journal.
The study shows that the bone structure of the jaw was sparse in around 20% of the women aged 38-54 when the first examination was carried out, and that these women were at significantly greater risk of fractures.

The study also shows that the older the person, the stronger the link between sparse bone structure in the jaw and fractures in other parts of the body.
The article “A prospective study of mandibular trabecular bone to predict fracture incidence in women: A low-cost screening tool in the dental clinic” was published in Bone in October.

What is the length of time for a cavity to get to the nerve?

The distance from the surface of a tooth to the nerve inside ranges from less than a millimeter to about one quarter of an inch depending on the type of tooth. As bacterial acid in the mouth dissolves the enamel surface of the tooth it  causes a growing opening  which is called a cavity. How fast this opening enlarges and travels a few millimeters depends on the amount of acid present, how intact the tooth has become as it  has grown to adult size, how many surface defects there are, and how many scars, abrasions  or fractures are in the surface enamel or dentin. A person who can’t brush or floss, who accumulates acid producing bacteria around the teeth, who does not use fluoride or oral calcium products, and who has stomach acid in the mouth from GERD or bulimia can grow cavities that invade the dental nerve in as little as three or six months. It is not unusual,  to see young people develop cavities from a lack of oral hygiene as soon as their teeth appear in the mouth. For some people the rate of decay is much slower, taking years or even decades to grow larger. You would think that all cavities hurt, but strangely enough, some shallow cavities hurt, and some deep ones do not. That is why dental exams every six months and periodic x rays are important, to prevent small cavities from reaching the nerve and requiring a root canal or removal. Prevention is worth a pound of cure when it comes to teeth, make sure you talk to your dentist about the many new technologies available (probiotics, liquid calcium, sealants, anti bacterial mouth rinses and toothpastes with and without fluoride) as well as flossing and brushing methods that preserve teeth and prevent cavities.

How long does it take for a cavity to hurt?

Should you wait till your tooth hurts?

Most people are amazed to learn that cavities start as microscopic pits. They result from an acidic imbalance in the normal chemistry around the enamel layer of a tooth. When the tooth is attacked by acid (usually produced by germs that live in the mouth) the surface layer dissolves. Gradually the tiny pit grows larger and larger until it finally reaches the soft inner part of the tooth called dentin.

This dissolving process can take many years, or sometimes may seem to happen almost overnight. Some times very small cavities are very sensitive, and some very large cavities are not at all. In either case, when the germs causing the cavity reach the far inside of the tooth (commonly called the ‘nerve’ or ‘pulp’), then an infection sets in which can spread to the jaw, or the face, or sometimes the lungs or the brain. (I spoke to some one last week who had part of her lung surgically removed because of bacterial infection that spread from her mouth.)

So, strangely, small cavities or large cavities sometimes hurt and sometimes don’t hurt at all. Sometimes patients come in with a large infection that has caused a remarkably big hole in their jaw bone that shows on their digital x ray, and the patient is not even aware that the infection or the hole is there. We look at the digital x ray together and they usually say “I don’t even feel it”. Usually though, an infection in the nerve is painful and obvious.

Most people prefer to fix smaller cavities early before they hurt or spread. Most people that do this keep their teeth longer with fewer problems than people who like to “wait until it hurts”. Since silver fillings are not very good for easily treating the smallest cavities, in the old days, before modern dentistry, dentists would wait, or ‘watch’ cavities while they grew. Why does my filling hurt?

After a trip to the dentist, for the next several weeks you may experience sensitivity to hot or cold which may last a few seconds. This is perfectly normal when your teeth are restored. If you have any sensitivity which lasts longer than a few minutes when exposed to cold or hot, or if there is sensitivity to biting pressure or the release of biting pressure, or there is sensitivity in the tooth on top or underneath the one treated, then the bite may have to be adjusted. This should be taken care of as soon as possible to avoid more lasting discomfort.
Modern tooth colored fillings are made of materials that blend into the tooth visually, so they may be very difficult to find and correct slight excess material in only one visit. Your mouth is sensitive to changes in your bite as little as fifty microns (.05 mm.) difference, so very slight changes can make a tooth uncomfortable.

Treatment may be something as simple as needing your bite adjusted. When your mouth is numb it may be difficult for you to bite the way you normally do, for this reason it may be impossible to get the bite exactly right. If so an additional adjustment may be all you need.

In some cases these symptoms may be an indication the tooth has a problem which may require additional treatment, especially if the symptoms last longer than a few minutes at a time.

Large fillings may not be adequate for some teeth to chew and feel comfortable. The tooth may have a crack or fracture in it which is sensitive, and cannot be repaired with a filling. In this case you may be advised to have a crown or other treatment (root canal). If this is the case, you should treat these teeth as soon as possible to avoid complications or avoid losing the tooth entirely.

Hormone Fluctuations and Gum Disease

Maintaining dental health when hormones change

Though women tend to take better care of their teeth than do men, they do not have significantly better oral health than men. In fact, women account for about 75% of office visits for periodontal care. Part of the unique challenge for women is that hormonal changes cause changes in oral health – directly affecting gum tissue and bone health, and often causing a host of uncomfortable symptoms.

In my practice, I’ve had the privilege of helping many women achieve and maintain not only overall dental health but also relief from the oral symptoms that hormonal fluctuations can bring on.

What are the life stage changes in hormone balance that may negatively affect your oral health? And what warning signs can you look for?

Puberty

During puberty, an increase in progesterone and estrogen causes more blood circulation to the gums, which may swell and bleed even though you brush and floss regularly at home. Gum tissue becomes far more sensitive at this time, more tender, and easier to irritate. This increased irritation tends to go away as puberty progresses; however, your dentist can certainly help if symptoms become bothersome at this time.

Monthly Cycle

Days before menstruation, you may notice red, swollen or bleeding gums, maybe a sore inside your cheek or a canker sore around your mouth. Thanks to a boost in progesterone, which dilates blood vessels and increases inflammation, some women experience gingivitis symptoms just before their periods or at ovulation. The good news is that these symptoms frequently clear up once the period starts, and there are ways of protecting gum tissue and lessening the irritation if symptoms reoccur every month.

Taking Oral Contraceptives

Swollen or bleeding gums commonly occur in women taking oral contraceptives. If your symptoms become uncomfortable enough to seek your dentist’s help, be sure to let her or him know specifically what medications you’re taking. Some antibiotics that dentists may prescribe for these symptoms actually decrease the effectiveness of oral contraceptives. So communication with your dentist is key.

Pregnancy

A higher level of progesterone can cause exaggerated gingivitis from approximately the second month to the eighth month of pregnancy. Though you take excellent care of your teeth, you may still find that your gums bleed when you brush. For some women, gum tissue becomes so sensitive that an irritant (such as plaque) can make it swell into a large, mostly painless bump (called a pregnancy tumor) that usually goes away within a few months after delivery or can be removed by your dentist if it becomes bothersome.

Whether or not you experience gum sensitivity during your pregnancy, make a dental hygiene visit part of your prenatal care, as periodontal disease has been linked to pre-term birth and low birth weight babies.

Menopause & Post-Menopause

During menopause, you may experience changes in the way things taste, be more sensitive to hot and cold foods, and feel as if your tongue and gums are burning. You may take new medications that cause dry mouth as a side effect. Most importantly, declining levels of estrogen can lead to loss of bone (osteoporosis and osteopenia), which includes the bone surrounding your teeth as well. Also, medications taken to prevent bone loss make more frequent cleanings necessary.

Why not treat these symptoms with oral care products from the store? Because some over the counter mouth rinses and toothpastes can actually aggravate these problems – and should be avoided.

United Nations High Level Session on Digital Development for Global Health

I was recently privileged to attend by invitation from Mr. Denis Gilhooly, Principal Advisor, United Nations Office for Partnership, Executive Director Digital He@lth Initiative, Co Secretary, Broadband Commission for Digital Development and Gerard McKeon of Black Tie Magazine, a High Level working session of the Digital He@lth Initiative titled: “Digital Health for Digital Development: Connecting the Millennium Development Goals and Non Communicable Diseases in 2011” at the United Nations Headquarters, New York on June 10, 2011.

The temporary buildings of the UN hold invaluable gifts from many nations hanging in hallways and resting in glass display cases transported from the main building which is under renovation. The transience of the surroundings did not mask the serious purpose of the session which focused on improving multinational health care via innovative communication technology consistent with the UN Millennium Development goals.

This singular summit brought together important top political and corporate figures to structure a global partnership of digital health through digital development. The approximately 45 participants included a deliberate mix of political and corporate representatives; Deputy Prime Ministers, Ambassadors, corporate Vice Presidents, and Senior Directors of pharmaceutical and technical companies. The attendees came from many countries including India, Ghana, Tajikistan, El Salvador, Rwanda, Commonwealth of Dominica, the offices of the Secretary General of the United Nations, the World Health Organization, UNICEF, Johnson and Johnson, Pfizer, Kaiser Permanente, GE Healthcare, Intel, Google, South-South News, and many others of great importance.

The Session Chair, H.H Amb. Francis Lorenzo, President of South-South News, former Deputy Permanent Representative to the UN and focal point at the Dominican Mission to the UN for work related to the United Nations Millennium Development Goals (MDGs) introduced the speakers.

The first highlight of the session came during remarks by H.E. Dr. Agnes Binagwaho of the Republic of Rwanda. Dr. Binagwaho revealed some remarkable statistics engendered by the incorporation of a single iPhone distributed to each of about 250 villages of approximately 150 persons. This phone had simple applications written to assist in reporting pregnancies and other medical events to a central station which then dispatched emergency care, or supervised chronic care for the village. As a result of this simple device, as well as other innovations, the mortality rate among women dropped from 8 per day to 1 per day, measured over a period of a few years. In addition, the per capita life expectancy in Rwanda has been dramatically increased from the mid 30’s to the mid 50’s in age.

Dr. Binagwaho used the humorous acronym BBC (born before computer) to describe herself, as many of the attendees nodded in agreement. She said that the simple graphical nature of the custom application helped to remove unfamiliarity of the computer and resulted in huge efficiencies in managing medical operations in Rwanda. I privately offered Dr. Binagwaho assistance in developing best practices in dentistry for her country.

Another highlight of the session was the speech by Dr. Vinton Cerf of Google, who foresaw cloud based digital medical records for universal accessibility. He mentioned that the technical aspects of delivering digital medical records and internet based medical services equaled or dwarfed the difficulties in developing purely software or hardware based applications.

Many other speakers including Dr. Scott Ratzan, Vice President of Johnson and Johnson, and Dr. Narayan Sundararajan, Director of Global Health, Intel, contributed to the free exchange of information about the challenges and benefits of the Digital Health initiative. Mr. Denis Gilhooly, who was our host, performed a superb job moderating the session with deep erudition and humorously calling some of the corporate attendees to task.

Acknowledged during the meeting were the impediments to technical implementation in developed countries, including the overhead of legacy systems, and the regulatory thicket that prevents simple initiatives from getting off the ground, but are no barriers to implementation in countries that have fewer restrictions due to their different prior political history.

In summary, a rare meeting at the UN brought together political and corporate figures invested in improving global health through digital innovation; demonstrated success with off the shelf technology; and initiated international cooperation to distribute the silo-ed wealth of medical know how across programs, technologies and points of care.

Halitosis (Bad Breath), more than a social problem

Ever have a conversation and find someone with offensive breath.

Now there are new reasons to let your friends know about halitosis. Oral bacteria have been linked to diseases like cancer, pre term birth, mental status, and (surprisingly) even weight gain. Bacteria in the mouth (the most common cause of halitosis) are easily removable by brushing, flossing and periodic professional cleanings.

Bacteria grow everywhere in the mouth, starting in small colonies and then differentiating into different niches called biofilms. As the colonies grow larger, deeper layers calcify and become hard to remove, unless a dentist or hygienist ‘scales or planes’ the layers off the teeth. More layers produce swelling that deepens the crevice between the tooth and gum. Eventually very toxic bacteria take hold and cause severe swelling, bone loss and will even burrow into the tissues and blood stream. The characteristic odor of halitosis is caused by bacterial byproducts called sulfides and mercaptans.

Cleaning away the bacteria removes the odor and eliminates the swelling so the gums get healthier, there are fewer places for bacteria to grow, and inflammation (the body’s fire alarm) stops. Daily brushing, flossing and cleanings every 3 to 6 months help keep bacteria in check.

When you alert your friends to halitosis, you are preventing the environment which can contribute to breast cancer, Alzheimer’s, atherosclerosis, and other terrible diseases. The fresh breath created by better oral care is a signal that disease is being kept away. Don’t ignore halitosis, do your friends a favor and let them know!

Case Study: Cancer and Dental Care

“Super” General Dentistry

Paul H. is a Registered Nurse and a Second Lieutenant in the US Army Reserves. Paul was diagnosed with head and neck cancer and received resective surgery and 40 radiation therapy treatments to his head and neck in 2001. Paul presented to my office with untreated rampant dental decay, class III mandibular anterior mobility and active generalized periodontal disease. He wished to keep as many teeth as possible to avoid post-operative extraction necrosis from the radiation treatment.

Because of his deployment to active duty the time available for treatment was limited to one year only. The treatment plan included caries control, oral hygiene and periodontal care and orthodontic leveling, alignment and correction of rotations, (but not full correction of class II), splinting the mobile teeth and extraction of non-restorable teeth. No esthetic treatment was performed at this time.

After consultation with his oncologist and radiologist, twenty pre-operative hyper baric oxygen treatments and ten post-operative visits to prevent radiation osteonecrosis of the mandible and maxilla were delivered at a hospital hyperbaric unit.

After extractions, caries control restorations, oral hygiene instruction and periodontal care, orthodontic treatment phase began March 24 2010, and ended February 17, 2011. Progress photos are attached. Paul is very pleased with this stage of treatment which has been uneventful and on schedule. He is cavity free, his periodontium has improved substantially, his teeth are stable and more maintainable and his overall appearance has improved dramatically. He is ready to begin esthetic modifications and replacement of the missing teeth.

The advent of modern medical care has resulted in patients with more complex treatment needing multispecialty involvement. A “super” general dentist may combine multimodal care in one clinical setting to make treatment simpler for patients with complex lives. This is one example among many of the advanced dental care my staff and I enjoy providing.

Fish Food Fighters vs. Gum Problems

In a  study with  9,000 adults,  higher dietary fish intake resulted in an approximately 20% reduction in gum disease.

Elizabeth Krall Kaye, PhD, Professor, Boston University Henry M. Goldman School of Dental Medicine, also notes that reduced odds of periodontal disease were observed at relatively modest intakes. Another  note was the suggestion that there seemed to be no further reduction in odds or periodontal disease conferred by intakes at the highest levels. Third, the results were no different when dietary plus supplemental intakes were examined. These findings are encouraging in that they suggest it may be possible to attain clinically meaningful benefits for periodontal disease at modest levels of polyunsaturated fat intakes from foods.

Foods that contain significant amounts of polyunsaturated fats include fatty fish like salmon, peanut butter, margarine, and nuts.

Could Acetominophen Ease Dental Anxiety?

A study shows Acetominophen eases Social Anxiety, could it also ease Dental Anxiety?

Association for Psychological Science

A research team led by psychologist C. Nathan DeWall of the University of Kentucky College of Arts and Sciences Department of Psychology has uncovered evidence indicating that acetaminophen (the active ingredient in Tylenol) may blunt social pain. DeWall and colleagues investigated this connection through two experiments. In the first experiment, 62 healthy volunteers took 1,000 milligrams daily of either acetaminophen or a placebo. Each evening, participants reported how much they experienced social pain using a version of the “Hurt Feelings Scale” – a measurement tool widely accepted by psychologists as a valid measure of social pain. Hurt feelings and social pain decreased over time in those taking acetaminophen, while no change was observed in subjects taking the placebo. Levels of positive emotions remained stable, with no significant changes observed in either group. These results indicate that acetaminophen use may decrease self-reported social pain over time, by impacting emotions linked to hurt feelings.

“We were very excited about these initial findings,” DeWall said. “The next step was to identify the neural mechanisms underlying the findings.”

In the second experiment, 25 healthy volunteers took 2,000 milligrams daily of either acetaminophen or a placebo. After three weeks of taking the pills, subjects participated in a computer game rigged to create feelings of social rejection. Functional magnetic resonance imaging (fMRI) employed during the game revealed that acetaminophen reduced neural responses to social rejection in brain regions associated with the distress of social pain and the affective component of physical pain (the dorsal anterior cingulate cortex and anterior insula). In other words, the parts of the brain associated with physical lit up in the placebo subjects when they were rejected, while the acetaminophen group displayed significantly less activity in these brain areas in response to rejection.

According to the academic paper detailing the experiments: “…findings suggest that at least temporary mitigation of social pain-related distress may be achieved by means of an over-the-counter painkiller that is normally used for physical aches and pains.

If acetaminophen reduces the distress of rejection…”, the stress of dental anxiety could be reduced as well.

Dentist, Staff, Bring Smiles To Special Patient

When dawn helt got the diagnosis of neck cancer recently she could have been depressed and overwhelmed. Even though at 63, she has struggled to make ends meet, suffered a stroke, and takes care of her disabled brother John, the Belford resident never felt anything but blessed. “I’ve got God on my side,” she says. “And good people.”

Helt, who has worked for McDonald’s for years, discovered she needed extensive - and expensive - dental work completed before she could undergo radiation treatment. A quick and less expensive alternative would be to have her teeth removed.

But when Dr. Robert Korwin, a Middletown dentist who has treated the Helt brother and sister for many years learned of Dawn’s situation, he wanted to help.

Over the next few weeks, Korwin and his staff worked through lunch hours, gave up personal time and provided Dawn with seven crowns, five restorations and $12,000 worth of treatment - at no charge to her. “It was important to keep her as healthy as possible,” he says.

“You know I always like to give something back to people, to let them know I appreciate them,” says Dawn, who gives generously in time and money to various charities. “Dr. Korwin and everyone here have been so good to me. I just love them all,” she says with a smile.

Flossing: So Easy, A Monkey Can Do It.

“Making sure your offspring know how to clean their teeth appears to be as important to monkeys as to humans.

Female monkeys in Thailand have been observed showing their young how to floss their teeth - using human hair.

Researchers from Japan said they watched seven long-tailed macaques cleaning the spaces between their teeth in the same manner as humans.

They spent double the amount of time flossing when they were being watched by their infants, the team said.

This suggests the mothers were deliberately teaching their young how to floss, Professor Nobuo Masataka of Kyoto University’s Primate Research Institute said.

“I was surprised because teaching techniques on using tools properly to a third party are said to be an activity carried out only by humans,” he told the AFP news agency.

He said the study, carried out in Lopburi, north of Bangkok, is still in the hypothesis stage.

“We would like to shift our focus to the baby monkeys to check whether the mothers’ actions are effectively helping them learn how to clean their teeth,” he added. ”

He declined any treatment without sedation. After consenting to an exam, he was guided toward therapeutic trust in the professional relationship. An exam with permission statements, retaining full personal control was the first step. After the exam, the  possibility of a cleaning emerged and the hygiene visit retaining full patient  control followed. Insight into Immune Response from Periodontal Bacteria

The mouth is telling us something!

A very significant finding for diagnosing and improving the immune system response to bacterial attack comes from periodontal research from The University of Louisville School of Dentistry, published in Science Signaling. The study reports that a bacteria responsible for periodontal disease interferes with the communication network of the human immune system, preventing destruction of the bacteria (P. gingivalis) by white blood cells. This immune interference is a highly sophisticated approach to keeping  the bacteria viable in the body. Many diseases which trigger  immune dysfunctions (such as Lyme for example) interfere with immune response in unknown ways.  The hope  is that this particular finding  may improve treatment of both periodontal disease and other systemic disease in a more effective manner.

Teeth on Fire

I’ve said it 3,000 times in the last 12 months. When I use a whitening or tartar control toothpaste, my teeth hurt! Don’t use these popular toothpastes. Instead use Arm and Hammer Age Defying and add MI Paste from GC. The combination is very powerful and tooth sensitivity melts away.

The simplified long winded explanation for this has to do with the anatomy, morphology  and physiology of a tooth. Oral Cancer Update

Oral cancer screening is something we have done every dental exam visit for the past 3 decades.

As reported in a recent interview with Michael A Kahn DDS of Tufts University School of Dental Medicine, only 15% of patients are aware of being given an oral cancer exam by their dentist. Since more people die yearly from this than from cervical cancer and melanoma, it should be on the top of your to do list. Risk factors include smoking and chewing tobacco, along with alcohol. The aldehyde in alcohol increase the absorption of cancer causing substances. Dr Kahn advises his patients, as I do, to avoid swishing daily with an alcohol based mouthwash. Perform a self exam once a month. Look for raised red or white patches in your mouth with a flashlight and a hand mirror. If present after two weeks, have us look at it, and possibly take a sample (biopsy) for analysis. Survival is high with early diagnosis and treatment.

Is there really mental floss?

Lose Weight!

Use the floss twice a day exercise.

When researchers in Brazil studied 13,000 people over a three-year period, they found that men with the highest levels of inflammatory agents in their body were also the most likely to gain weight!
Periodontal disease, which is caused by poor oral health care, is one of the most common sources of inflammation.
Brushing twice a day, flossing, and making regular trips to the dentist are the best ways to prevent the disease.

Can you tell your stomach not to growl?

You can’t stop the rain, peristalsis, or tooth grinding.

Everyone grinds their teeth at times but not everyone knows they do it. Even if they do, it can be hard to stop.

To eat a steak, your teeth have to pound away with about 45 pounds of pressure. Studies show that teeth are in contact only 5 minutes per day while eating. On the other hand, while clenching or grinding, a much greater force is used for a much longer time. Your jaw muscles can produce a peak force of 200 to 300 pounds when clenching, enough to break a tooth.

Since teeth crack at 125 pounds of pressure, those 200 pounds repeated for 6 to 8 hours each day produce visible fractures, loss of supporting bone, and abfractions. Usually mistaken for overzealous tooth brushing, abfractions resemble a sharp horizontal notch cut into a tree with an axe. (Heavy tooth brushing produces root wear that is dished out and wide, not sharp).

For the last 30 years, when I find patients with tooth fractures, excessive wear and abfractions, I always mention it. I show live videos of their teeth and explain the evidence. To my surprise, after seeing this, a few people will say they never grind or clench. I can watch people while they tell me they never clench or grind… in the act of clenching and grinding. An observer can watch their jaw muscles flex and pop while I inform them they are doing it. I hand them a mirror, point it out, and then the conversation stops and there’s a pause in the conversation. Never fails.

Those who don’t demonstrate it on cue, will return 4 weeks later and say…’you know doc, I grind my teeth when I’… and a variety of answers emerge. It is during stressful family or workplace conversations, while driving, while arguing, etc. This scenario happens so often, I now tell some patients to tell me in one month whether they clench or grind.

The fact is that we are always engaged in sub conscious homeostatic activities like digestion that are not supposed to be conscious, until dysfunction clues us in. So the moral of the story is… damage is the result of clenching and grinding, but we are sometimes completely unaware that we are doing this.

How can you tell if a Dentist is excellent?

Is there a measure of dental excellence? .

Some people ask: does he hurt?; is he good with kids?; do his fillings fall out? Some people send in the kids and if they like the Dentist he is ok.
I make it mandatory for everyone in my office to be very concerned with being gentle, kids like me, and my fillings don’t fall out.
There’s another way to tell, though. In my office, a root canal after a filling that I do is necessary only 1% of the time, and only 3.8% of the time when a crown is needed*. (Crowns are done when a tooth is more severely damaged in the first place).

But the “1%” number is a rigorous test. This means that almost all decayed or damaged teeth needing fillings and crowns done in my office don’t need root canals. My staff informs me that in some other offices the percentage is as high as 10% to 50% of the time. If you had a choice, I’m sure you’d rather have just a filling or crown, rather than a filling and root canal. The question and measure of dental excellence is to ask: …’do your fillings and crowns turn into root canals?’ If they don’t turn into root canals, you are in the right place.

Canker Sores and B Vitamins

“A team of physicians at Ben-Gurion University of the Negev has discovered that a nightly dose of vitamin B12 is a simple, effective and low risk therapy to prevent Recurrent Aphthous Stomatitis (RAS), better known as “canker sores.” Canker sores are one of the most common oral lesions. According to lead researcher Dr. Ilia Volkov, “the frequency of RAS is as much as 25 percent in the general population, however, until now, there has been no optimal therapeutic approach.”
The findings were reported in the January/February issue of the Journal of the American Board of Family Medicine 22 (1): 9-16 (2009). Dr.Volkov is a primary care physician in the Clalit Health Services and lecturer in Ben-Gurion University’s Department of Family Medicine in its Faculty of Health Sciences. The researchers tested the effect of vitamin B12 on 58 randomly selected RAS patients who received either a dose of 1,000 mcg of B12 by mouth at bedtime or a placebo, and were tested monthly for six months. Approximately three quarters (74 percent) of the patients of the treated group and only a third (32 percent) of the control group achieved remission at the end of the study.
According to the research, “The average outbreak duration and the average number of ulcers per month decreased in both groups during the first four months of the trial. However, the duration of outbreaks, the number of ulcers, and the level of pain were reduced significantly at five and six months of treatment with vitamin B12, regardless of initial vitamin B12 levels in the blood.
During the last month of treatment a significant number of participants in the intervention group reached ‘no aphthous ulcers status’ (74.1% vs 32.0%; P < .01).” The treated patients expressed greater comfort, reported less pain, fewer ulcers, and shorter outbreaks during the six months while among the control group the average pain level decreased during the first half of the period but increased during the second half.”

Listening is a bilateral agreement

We all know doctors should listen, but do patients want to listen to doctors when denial feels better than the reality of a disease.

No one wants to know that they are sick, but sometimes, the comfort of denial makes a person so sick they can’t recover. During my residency program, I was among a small group of dentists who were introduced to a nicely dressed, well spoken man of about 50 years, and his 25 year old son. Our professor encouraged us to meet him and interact. I naively enjoyed the conversation, and we left the room after examining this man’s chief complaint… a large swelling under his tongue.
Out of earshot, a few minutes later, the professor quietly told us the gentleman would live approximately another two weeks. He had ignored his dental problem until he was completely unable to eat, and the cancer had spread everywhere. The information shocked us. The professor went on to say that the tumor had been operable in the early stages, but was completely untreatable now.
I felt sick, and sorry for a man who had the opportunity of health, and had never sought it out.
Since then, I perform an oral cancer screening every 6 months for all my patients, and I think of this gentleman. His legacy to me is, try to seek the beginnings of problems and prevent them. And keep trying to communicate, even when patients turn a deaf ear. Statistics show that information needs to be repeated up to seven times before it is retained. I hope that at least once is early enough to help someone else from ending up like that poor fellow many years ago.

Air Abrasion

Wow, that was quick.

Air abrasion was invented in the 1950’s, but did not become used in dentistry until the invention of more durable bonding materials which could be easily placed into smaller cavities in the 1990’s. The silver amalgam used in the 1950’s could not be used with air abrasion since it does not stick to the tooth. The techniques of minimally invasive dentistry and air abrasion are used to treat only the cavity and nothing else in the tooth. Once infected tooth decay is removed by the air abrasion stream of aluminum oxide, the tooth opening is then chemically prepared, sealed and then filled with a durable adhesive tooth colored composite. The whole process is painless and is very durable. The tooth is less likely to be sensitive than a silver filling after treatment, and the tooth is stronger since less structure is removed than with traditional drills and silver fillings. This is a fast, painless and lasting method of fixing a problem tooth. I use it for almost every filling since it super cleans the tooth faster and more effectively than any other method.

Excessive acid in the mouth can lead to more cavities. You can do an a home test with a pH strip (ask us for it, or buy one in most drugstores): A reading lower than 6.5 indicates unhealthy levels of mouth acid found in your saliva. Tooth enamel will dissolve at a pH of 5.5. To balance out pH levels, increase your calcium, magnesium, and potassium intake and brush with a pH-neutral toothpaste, which won’t cure the problem but will minimize the damage. Why Does My Tooth Hurt After a Filling?

Like other parts of the body, if teeth are injured, there are nerves that say ouch.

When you have a cavity that has developed in your tooth, the tooth nerve responds. It can become hyper sensitive and say ouch more quickly, and last longer . A cavity is produced by an infection in the tooth. The acid produced by the germs eat away at the inside until a hole develops.
A filling really is like a biopsy on a tooth. The bad part is completely removed, (we don’t send it in for analysis though), and an artificial replacement is put back. The dentist’s skill combining art and science, makes the tooth look like new right away. Like any body injury, though, the tooth longer to recover, and the nerve, which may be slightly damaged, may be sensitive for a time. As it repairs, the tooth gradually feels better.
Sometimes the nerve doesn’t recover and then the tooth is treated with a root canal or extraction.
Preventive dentistry is the science of repairing problems before you have a toothache or tooth sensitivity. Treat cavities when they are small and the chance of a tooth ache, root canal or extraction drops to near zero.

Why use alcohol containing mouthrinses when there are effective non alcohol mouth rinses available OTC?

Some studies have pointed to increasing evidence that alcohol based mouth rinses may be harmful when used repeatedly.

Ethanol in mouthwash is thought to allow cancer-causing substances to permeate the lining of the mouth more easily and cause harm.
Acetaldehyde, a toxic by-product of alcohol that may accumulate in the oral cavity when swished around the mouth, is also believed to be carcinogenic.

First all of the plaque is cleared away and the shallow cavity is cleaned off with a mild acid solution. The tubule dental system opened by the cavity is then dried with ethanol and a thin resin solution is applied, penetrating the tubules and closing them. Excess is removed and a light used for tooth colored fillings sets the material to a tooth like hardness. This 15 minute procedure strengthens the tooth comfortably. A higher level of progesterone can cause exaggerated gingivitis from approximately the second month to the eighth month of pregnancy. Though you take excellent care of your teeth, you may still find that your gums bleed when you brush. For some women, gum tissue becomes so sensitive that an irritant (such as plaque) can make it swell into a large, mostly painless bump (called a pregnancy tumor) that usually goes away within a few months after delivery or can be removed by your dentist if it becomes bothersome.

Whether or not you experience gum sensitivity during your pregnancy, make a dental hygiene visit part of your prenatal care, as periodontal disease has been linked to pre-term birth and low birth weight babies.

Although Women tend to take better care of their teeth than do men, they do not have significantly better oral health than men do. This increased irritation tends to go away as puberty progresses; however, your dentist can certainly help if symptoms become bothersome at this time.

Oral Cancer is a disease that can be tamed. If the disease is discovered in late stages, the 5-year survival rate is only 20%-to-30%. But when discovered in early stages, the survival rate leaps to 80%-to-90%. By supplementing the white light oral cancer dental exam with the VELscope system, I can see cancerous and precancerous lesions that might not be apparent to the naked eye. Overcoming Dental Anxiety Organically

When you think of the dentist how does your body react?

Why take Blood Pressure at the Dentist?

Out of sight, out of mind.
Our goal is to help our patients maintain and stay healthy. With a busy lifestyle, monitoring blood pressure can sometimes take a back seat. Elevated blood pressure causes systemic disease and shortens lifespan. When we take our patients blood pressure we can flag changes, and refer to a medical professional to help control the problem. Taking blood pressure (at no charge) on all our patients has helped us find many, many, cases of previously unknown elevated pressure. Few dentists do this, but we all should. Some patients have “white coat syndrome” or anxiety when they go to any dental or medical office. In most cases elevated pressure of 10 mm or less can be a healthy response to anxiety. When pressure is elevated by 30 to 50 or more in some cases, then a serious underlying condition is present that should be medically treated. In some cases blood pressure can be so high that dental procedures cannot be provided until the pressure is lowered. Screening for blood pressure is a dental provider’s responsibility and a community service we have provided for decades.

Teeth Whitening

Almost all teeth, even the darkest ones, can be whitened.
Whitening occurs when a peroxide (carbamide or other) is applied to stained areas. Just like bleach in the washing machine, stains are cleared. The darker the tooth color, the longer the whitener must be applied. To minimize sensitivity, the percentage of peroxide is variable, as well as the amount of time the whitener is applied to the teeth. We also use a desensitizer before the whitener for maximum comfort during the treatment. Techniques that involve paint on, tray applied, and light or laser boosted, are all effective. The secret is in the amount of peroxide, the length of time it is applied, and how effective the application technique is. Power whitening is a combination of home and in office whitening. The results are dramatic. We usually whiten teeth after Invisalign for beautiful cosmetic results.

Air Abrasion is Fast, Effective, and Painless
Quick, pain free, conservative and complication free, I have been using air abrasion technology without the need for numbing the tooth for two decades. Sometimes, the cavity is so deep, I wonder to myself, why doesn’t it feel uncomfortable, but it is really painless. I always ask the patient to tell me if there is the least bit of discomfort. They say to please continue, and amazingly, we finish the procedure and the patient is happy and leaves without the saggy lip appearance typical of local anesthetic. I wish I had this for myself when I was younger. It is the dentist’s and patient’s dream procedure.

Dry Mouth- The Undiscussed Arid Zone

Avoiding New Cavities
Dry mouth will cause cavities even when you are the best brusher ever. Some patients will develop a whole crop of new decay, especially when they are taking new blood pressure or other systemic medications. Let your physician know if you are getting recent new cavities. This problem is very avoidable with proper diagnosis. Don’t be a stranger to your dentist, you are always welcome.

Laser Gum Treatment

New non surgical gum treatment with a laser is fast and painless.
Some gum problems can be treated with an high tech laser. During this radically advanced care, the bacteria causing the gum infection are vaporized, leaving a disinfected space for the body to heal more rapidly and more effectively. The scarred lining of the crevice or pocket is eliminated as well, allowing healing to proceed more rapidly than with any other technique. Although some gum problems are too advanced for this therapy, mild to moderate periodontal problems respond very well. We have witnessed 100% improvement in some treatments with this rapid, comfortable process.

Unnecessary Root Canals or Extractions

A toothache is not always from a tooth.
Of about 50 patients who had appointments for dental pain in the last 4 weeks, half of the patients were convinced they needed a root canal treatment. A complete diagnosis was undertaken. This included a full history, X- ray, review of prior x rays, palpation of the area, occlusion (or biting) slight percussion (tapping), a gentle air stream, vitality testing, photography for fractures, and comparison to 30 years of similar clinical histories.
Dental X- rays usually showed cloudiness or calcification of the maxillary sinus on one or both sides. This together with marked tenderness in the maxillary tuberosity distal to the root of the last molar indicates a history of sinus inflammation. When the lining of the sinus is thickened from chronic allergies or infection, the ostium (or opening) which drains fluid from the sinus becomes clogged like a stopped up sink. The fluid does not drain, it produces pressure in the small sinus, and the pressure produces pain in the nerve endings within the sinus. This fluid can then become secondarily infected. The roots of the upper teeth are commonly in or near the sinus, so pain in the area is referred, or seems to come from these teeth. The pain actually comes from the sinus itself. The thin bony wall of the sinus expands under pressure, the teeth change position slightly, and then the lower teeth start to ache as well. The correct treatment should be directed to the sinus area. Clenching and grinding your teeth when they are aching makes the problem feel worse and harder to diagnose. Pathognomic for sinus infection diagnosis is a history of increased pain when using stairs or when bending the head down and up. Root canal pain usually doesn’t change with head position, whereas sinus pain changes with fluid level changes (like a carpenter’s level).
Since it feels like a toothache, the dentist is the first call. Without time spent diagnosing, everyone will agree a root canal should be done. Teeth that are intact, cavity free and not fractured might be root treated or even extracted. Unnecessary root canal treatment can be avoided by thorough diagnosis and if appropriate, referral to an Ear Nose and Throat physician. Clenching pain can be treated with a properly fitted professionally made night guard or bruxing appliance.
With proper management the problem will resolve, and the patient soon feels better. New Jersey is a prime area for sinus involvement, and these changes are clearly visible on a large percentage of patients x-rays.

The Dental Detective: Complex Problems Take Time to Uncover

A dental problem can be misdiagnosed. It occasionally may take time to find the true cause.
Patient C.. was confused and uncomfortable with facial pain that seemed to shift from left to right and come and go. It was so uncomfortable that she consulted 5 dentists and 2 physicians to no effect. She had multiple root canals and even resorted to extracting some teeth without pain relief. C.. was then referred to me by a relative who was a patient in our practice. When C.. arrived, she was pale and sleepless and maximally uncomfortable. After dental examination, records and diagnosis, the proper solution became apparent. Her teeth were slightly misaligned and she was unconsciously clenching her teeth. The excess pressure of clenching together with her misaligned bite was making her jaw muscles spasm, causing the discomfort.  After reviewing the source of the problem, she was instructed to reduce daytime clenching, her bite was smoothed in the appropriate locations and within 2 weeks C.. had  a 95% reduction of pain. At a reevaluation, her facial color was back to normal and she reported sleeping well again for the first time.
